Please provide a short bio about yourself (to remain confidential).

Please describe your “wish”. For example: you would like to be a character in your favorite television show, or travel to a foreign country with a best friend, or see a long-lost friend. Give as much information as you like.

Guidelines: 

1.       Please only provide first names for real persons who will appear in your story.

2.       Please provide enough detail information so I can better personalize your wish fulfillment story.

3.       The completed story will not contain any of the following: sexual content, immoral behavior, bad language, or describe harm to others.

4.       The completed story will be sent to you for approval prior to posting along with a form for you to sign giving me permission to post your “Wish Fulfillment” Story on my webpage.  Visitors to the website will be allowed to leave comments that will be posted online (pre-screened by me).

5.      If you are under 18 you will have to get your parent’s permission (their signature) on the authorization form.
